First of all you need to install Contact Form 7 plugin and than to import our demo content and you would get all the pages including desired one with desired contact form. However, if you do not want to import demo content you can quickly create such contact form with code that we will provide you here. So please go to Contact > Add New enter the title of the Contact Form and just in the editor of the contact form please paste this code:
