    Hi,

    Just use this code for that form. First install/activate Contact Form 7 plugin then http://screencast.com/t/8qkuT0YM

    then go to your admin panel Pages -> That page -> Add Contact Form 7 shortcode into your content and inside Contact Form 7 shortcode options you will find Custom Style option -> Set Custom Style 1 for example and save. Then go to your admin panel Elated Options -> Contact Form 7 -> Custom Style 1 and set your style for your form and save

    I don't now what was in the screenshot, it was a year ago. But as I can see ,you don't need screenshot, just follow the explanation bellow. Also please open a new ticket and send us URL to your website if you still have same issue.
